You can send your patient’s prescription to us in five ways:

Please fill out the Accredo HGH Certification Required form and fax the completed form to the Accredo Growth Disorder Pharmacy Team at 888-355-6682.
This form is required for ePrescribed, verbal, or non SMN-containing prescription formats for somatropin products, Increlex® and Egrifta® upon first fill for these drugs.

To ePrescribe using our prescriber website:
- Log in or register at MyAccredoPatients.com.
- Select “Electronic Referral” icon or navigate to Resources > iAssist
- You will be routed to iAssist, Accredo’s electronic referral service1 and be prompted to login or register
